NHA community health centers accept most insurance, including Medicaid and Medicare. Health care assistance on a sliding fee scale is available for those who are underinsured or uninsured.
Opened in October 2016, Nexus Health Care is the latest addition to the Neighborhood Health Association network of healthcare providers. Services provided at the 43,000-square-foot medical center include:
- Adult primary care (Internal Medicine, Family Practice)
- Cardiology (in partnership with the University of Toledo College of Medicine)
- Dental care (adult and pediatric)
- HIV testing
- Immunizations
- Laboratory services
- Midwifery
- Doula (for all pregnant patients of NHA)
- OB/GYN
- Pediatrics
- Pharmacy services
- Behavioral Health
- Podiatry
- Sickle cell detection and education

In addition, Nexus Health Care features an on-site café called Nourish Café. The site also houses the Mildred Bayer Clinic for the Homeless and offers numerous support services such as:
- Community liaison specialists
- Family planning
- Perinatal outreach
- Transportation
- Strong collaborative relationships with area shelters to ensure residents access to medical and dental services
All services are available to all NHA patients regardless of where they receive their care. No one is turned away from care, regardless of their ability to pay.

Monday – Friday: 8:00 a.m. – 4:30 p.m.
Summer Hours
First Monday in May through the last Friday in August
Monday – Thursday: 8:00 a.m. – 5:30 p.m.
Friday: 8:00 a.m. – 11:30 a.m.
The clinic offers access to bus lines, helping fulfill NHA’s mission of providing quality care for the entire community. Parking is available in the NHA parking lot directly across 14th Street.
Patients who are seen by NHA providers can have labs done at NHA. Any patient of NHA who has a lab order from an outside health system may have services done at NHA. Onsite lab services are located at most of the clinic locations. Lab hours are the same as clinic hours.
