NHA community health centers accept most insurance, including Medicaid and Medicare. Health care assistance on a sliding fee scale is available for those who are underinsured or uninsured.

FINANCIAL ASSISTANCE

Neighborhood Health Association (NHA) is committed to serving the needs of its patients and provides a SlidingFeeSchedule.pdf to assist both uninsured and under-insured patients.

MINIMUM FEE FOR SERVICE

If you are uninsured or underinsured patient at NHA, you may be eligible for a discounted rate based on the current Federal Guidelines established to assist you.

Please remember: If you need assistance, bring your two most recent payroll stubs to your appointment and identification with you.

Payment for services is expected prior to seeing the doctor at the time of service; the minimum co-pay is due each time.
